There is no need to miss work or school. Our practice offers care on select Saturdays. Open on Monday thru Thursday 8:00am-5:00pm, Fridays 8:00am-4:00pm.
Appointments Available
Avoid extensive wait times at other facilities. Same-day appointments may be available, along with E-visits. DOT/CDL visit is approximately 30-45 minutes. Primary Care visits vary from 15-45 minutes, depending on nature of visit.
Competitive Rates
Primary Care visits: Establish Visit- $125, Follow-up Visit(s)- $75, Procedure Visit- $50-150. Quest Labs: $75-225. DOT/CDL Exams: Current CDL Holders- $125 (Cash)/$130 (CC). Student- $100 (Cash) / $105 (CC). $50 deposit when scheduling. Drug testing upon request- $25. Sports Physicals: $25. Insurance is also accepted: Aetna, Cigna, BCBS, Presbyterian, United Health Care, Medicare, Medicaid. Please call for the most current list of accepted insurance providers.

What are your business hours?
SEA Family Healthcare, LLC is open Monday-Thursday 8 am-5 pm, Friday 8 am-4 pm. We are closed daily from 12-1pm for lunch (Mon-Fri).
What do I need for my Sports Physical?
Your school will require specific paperwork for you and your parent to complete. Please bring completed paperwork with you to your appointment. If needed, there is a link below for access to the most requested paperwork. If you wear glasses, please bring them as they will be necessary for your vision exam. Each exam is $25.
What do I need for a DOT/CDL Exam?
Please complete the required Medical Examination Report Form, MCSA-5875 prior to your visit & bring it with you for further review. You & the provider will discuss your medical history & complete your exam. Please know the medications you are taking & if needed, bring doctor clearnace letter for high BP, CPAP records. Height, weight, blood pressure, pulse rate, breathing, vision (bring glasses) & hearing will be checked. A urine sample to check for sugar, blood, protein, & specific gravity will be required.
